Hooks Family Practice Return & Refund Policy


Our Promise

At Hooks Family Practice, we stand behind the quality of our products and services. Your satisfaction and trust are our top priorities. If for any reason you are not completely satisfied with your purchase, we’re here to help.


Supplement & Product Returns

Because our supplements and wellness products are ingestible and health-related, we cannot accept returns or exchanges on any opened, used, or tampered items for safety and quality reasons.


We do, however, accept returns on unopened and unused products within 14 days of purchase, provided that:

    •    The seal is intact and the product is in its original packaging.

    •    Proof of purchase (receipt or order number) is provided.

    •    The product was purchased directly from Hooks Family Practice (in-office or through our official website).


Return Shipping:

Customers are responsible for return shipping costs unless the return is due to an error on our part (e.g., damaged, defective, or incorrect item).


Refunds:

Approved refunds will be processed to the original payment method within 7–10 business days after we receive and inspect the returned item.


Services, Consultations & Memberships


All consultations, visits, memberships, and telehealth services are non-refundable once rendered or activated.

If you need to cancel or reschedule a visit, please provide at least 24 hours’ notice to avoid a cancellation fee.


For Direct Primary Care Memberships, cancellations must be requested in writing with 30 days’ notice. Any prepaid services or add-ons will not be refunded after activation but may be credited toward future visits at our discretion.


Damaged or Incorrect Orders


If you receive a damaged or incorrect product, please notify us within 3 business days of delivery at:

support@hooksfp.com or (321) 468-8706  Include your order number, a photo of the product, and a brief description of the issue. We’ll promptly resolve it with a replacement or refund.

    •    Services, memberships, and consultations
