Hello! I’m Fanny Hooks, a Board-Certified Family Nurse Practitioner, and the proud founder of Hooks Family Practice. I established this practice to bring compassionate, comprehensive, and personalized care to my community—care that truly makes a difference in people’s lives.

I have over a decade of healthcare experience, from working in critical care and intensive care units to serving as a primary care provider for diverse populations. My background includes extensive training in family medicine, pediatrics, women’s health, adult-gerontology, and chronic disease management. 

 In addition to my nursing and nurse practitioner experience, I bring over five years of pharmacy technician experience, which gives me a unique perspective on medication management, patient safety, and pharmaceutical care. This background enhances my ability to guide patients through complex medication regimens and ensure they receive comprehensive, informed care at every visit.

My passion for healthcare is deeply personal. After experiencing my own life-changing journey—losing over 80 pounds through dedication to wellness and lifestyle change—I became even more determined to help others take control of their health. I believe in meeting patients where they are, whether that means telehealth, or home visits.
